Using two successive pairs of specialized CT scans, a team of Johns Hopkins and Dutch radiologists has produced real-time images of liver tumors dying from direct injection of anticancer drugs into the tumors and their surrounding blood vessels. Within a minute, the images showed whether the targeted chemotherapy did or did not choke off the tumors' blood supply and saved patients a month of worry about whether the treatment, known as chemoembolization, was working or not, and whether repeat or more powerful treatments were needed.

The Johns Hopkins team's report about this novel use of dual-phase cone-bean computed tomography, or DPCBCT, an imaging technique developed at Johns Hopkins, is set to appear in the January 2013 edition of the journal Radiology. The diagnostic scans were performed on 27 men and women with inoperable liver cancer.

"This new scanning method is giving us almost instant feedback about the value of injecting antitumor drugs directly into large liver tumors and their surrounding blood vessels in an effort to quickly kill them, and to prevent the cancer from spreading," says senior study investigator and interventional radiologist Jean-Francois Geschwind, M.D.

Geschwind says if further testing proves equally successful, the paired use of cone-beam CT scans, which are already approved for single-scan use by the U.S. Food and Drug Administration, could supplant the current practice of MRI scanning a month after chemoembolization to check its effects.

"Patients should not have to endure the uncertainty of waiting weeks or more to find out if their chemoembolization was successful in fighting their liver cancer," says Geschwind, a professor in the Russell H. Morgan Department of Radiology at the Johns Hopkins University School of Medicine and its Kimmel Cancer Center.

"Dual-phase cone-beam CT avoids such delays, which also could allow the cancer to grow and spread and, ultimately, compromise chances of remission," he says.

Avoiding delays is particularly important, he says, for people with moderate to advanced stages of the disease, when liver tumors are too large or too numerous to surgically remove, and for whom chemoembolization is the main treatment option. Half of such liver cancer patients succumb within nine months, and liver transplantation is only an option for a quarter of those whose tumors have not spread outside the liver.

The newer DPCBCT scans, in which X-rays are detected by a device the size of a large laptop that can be placed directly below or above the operating room table, have the added advantage of being performed in the same room, or interventional radiology suite, as patients getting chemoembolization.

In their new study, Geschwind and his colleagues found that the initial shrinkage seen with DPCBCT scans taken before and after chemoembolization matched up almost perfectly with MRI scans taken a month later. Tumor death was 95 percent, the same as that seen by MRI. A total of 47 tumors were closely monitored in the study to assess how well DPCBCT tracked tumor death after chemoembolization.

All study participants were treated at The Johns Hopkins Hospital between March and December 2009.

In DPCBCT scanning, a chemical contrast dye is injected into the artery that supplies blood flow to the liver and tumor right before the chemotherapy drug is injected, to enhance the X-ray image. The first set of scans highlights key blood vessels feeding the tumor, as dye flows in and out of the tumor. The second set of scans is performed immediately after chemoembolization, to gauge tumor and key blood vessel death. Computer software is used to sharpen and analyze differences between the images.

The entire DPCBCT scanning time, researchers say, is between 20 seconds and 30 seconds, and the total amount of radiation exposure from the dual scanning averages 3.08 milliseiverts, which is less than half the amount of radiation involved in a modern abdominal 64-CT scan. Cone-beam CT scanners also emit an X-ray, but unlike other CT scanners, the cone-beam type of X-ray is projected onto one large, rectangular detector, roughly a foot and a half long -- and produces a telltale conical shape. The size of the cone-beam CT detector allows for single scans that can capture images the size of most people's entire liver. More powerful 64-CT and 320-CT scanners involve multiple detector rows.

Chemoembolization entails the use of tiny beads containing the chemotherapy drug doxorubicin injected directly into liver tumors.

Ultrathin catheters, about the width of a human hair, are threaded through blood vessels to deliver the drugs, which seep from the beads for several weeks.

Geschwind is leading clinical trials under way at Johns Hopkins and other centers to assess whether the combination drug treatment works for liver cancer patients. Early results have shown promise, with patients with advanced disease living 10 months to 15 months longer.

Geschwind says they plan improvements in image quality in DPCBCT scans, hoping further refinements will encourage physicians to adopt the technique. They also plan updates to the navigational software that, like GPS, can track blood vessels feeding each tumor, and provide more precise and greater numbers of targets.

Liver cancer kills nearly 20,000 Americans each year, and is much more prevalent outside the United States, where it is among the top three causes of cancer death in the world. Experts cite the rising numbers of hepatitis C infections, which cause chronic liver inflammation and are a leading risk factor for liver cancer.

University of Maryland Terrapins: Mitchell leads Maryland past Delaware State 79-50

COLLEGE PARK, Md. (AP) - Freshman Charles Mitchell came off the bench and scored a season-high 19 points and Maryland extended its winning streak to 11 games by beating Delaware State 79-50 Saturday.

Dez Wells and freshman Seth Allen each scored 10 points for the Terrapins (11-1). Mitchell also had 14 rebounds for his third double-double.

Maryland has not lost since falling to then-No. 1 Kentucky 72-69 in the season opener. The current winning streak is Maryland's longest in 10 years.

Tahj Tate had 20 points for Delaware State (5-8), which has lost three consecutive games.

The Terps came in holding opponents to 34.6 percent shooting from the field, ranked third nationally, and they continued that trend by holding Delaware State to 32.1 percent (18 of 56).

After the Terps trailed early, a jumper by Mitchell capped a 17-2 run and gave them a 32-16 lead with 4:43 left in the half. A layup by Jake Layman at the buzzer gave Maryland a 40-22 lead, with freshmen accounting for 26 of those points. The Terps outscored the Hornets 32-10 inside the paint.

Maryland continued to pull away in the second half, and a layup by Wells increased the lead to 54-31 with 13:30 left. The Terps coasted the rest of the way.

Maryland has one more nonconference game Tuesday against IUPUI before opening Atlantic Coast Conference play Jan. 5 against Virginia Tech.

    Two new orchid species found

    One of the world's newest orchid species is also its most delicate, with tiny white flowers smaller than a dime. Yet the flower finds its home amid boulders near the banks of rushing streams in Cuba's remote eastern mountains.

    The orchid is one of two new species identified by botanists in Cuba, a hotbed for orchids ? the largest and most diverse plant family in the world. The islands of the Caribbean have more the 25,000 species of orchids tucked into their forests and rivers.

    The new species was named Tetramicra riparia, a nod to its discovery along stony streams in the mountains of Baracoa, one of the rainiest and least explored areas in Cuba, ?ngel Vale, a researcher at the University of Vigo in Spain, said in a statement. The plant has an unusually broad, sturdy base: Its pedicel is almost four times as large as its column, Vale and his co-authors report.

    The second new orchid, from the western tip of the island, dwarfs its neighbor in size. The flower's showy purple and green petals are similar to a daffodil in appearance, spreading more than 2.5 inches, with up to 20 blooms on one plant.

    Like many orchids, the flower, dubbed Encyclia navarroi, is epiphytic, meaning it grows on other plants for support, but not for nutrients. Along the western coast, the species preferred to perch on plumeria and ficus, the researchers discovered.

    Both new species are deceit pollinators, Vale said, enticing bees to spread their pollen without a reward. "Contrary to most plants, many orchids do not produce nectar or other substances to compensate insects and birds that visit them," he said.

    Vale and his colleagues are studying orchids throughout the Greater Antilles (Cuba, Jamaica, Hispaniola, Puerto Rico) to reconstruct their evolutionary history and analyze the effect of pollinators in their development. One of the mysteries they aim to solve is whether deceit orchids have greater diversity than other nectar-producing species.

    "Despite the fact that T. riparia's flowers have a complete central petal, just like other species that make up a subgenre endemic to Cuba; the way they grow is very similar to a more widespread group that seems to have diverged on the neighboring island of Hispaniola," Vale said. "Our work provides molecular evidence of the greater relationship of T. riparia with these species on the neighboring island."

    The findings were detailed in the October 2012 issue of the journal Systematic Botany and the April 2012 issue of the journal Annales Botanici Fennici.

    UK's Pearson invests in Barnes & Noble's Nook

    LONDON (AP) ? Pearson, the U.K. publisher and education company, is to take a 5 percent stake in Barnes & Noble's NOOK e-reader as technology companies seek new inroads into the potentially lucrative business of digital textbooks for schools.

    Pearson PLC will pay $89.5 million cash for a 5 percent stake in NOOK Media LLC which includes the bookseller's e-reader and tablets, its digital bookstore and its 674 stores serving U.S. colleges. Barnes & Noble will hold 78.2 percent of the business and Microsoft will have about 16.8 percent, the company said Friday.

    Major tech companies have looked for inroads into the industry, seeing tablets like the iPad and the NOOK as replacements for the dozens of books that students must lug to and from school each day.

    Walter Isaacson in his biography of Steve Jobs wrote about meetings between the co-founder of Apple Inc. and major publishers to accomplish just that before his death last year.

    Janney Capital Markets described the tie-up between Pearson and Barnes & Noble as an "online education dream team."

    "After this investment from Pearson, it is more clear that Nook Media has its sight set on transforming the way education is administered in the U.S. and around the world," analyst David Strasser wrote.

    Pearson is the largest higher educational publisher in the world and the largest in kindergarten-through-high school publisher in the United States, Janney said. The company, which also owns the Financial Times and whose Penguin book brand is in the process of being merged with Random House, reported that its textbooks and training made 1.9 billion pounds ($3 billion) in revenue the first six months of 2012.

    Will Ethridge, CEO of Pearson North America, says his company had worked with Barnes & Noble for decades and have invested heavily in providing engaging and effective digital reading experiences.

    "It is another example of our strategy of making our content and services broadly available to students and faculty through a wide range of distribution partners," Ethridge said.

    Barnes & Noble Inc. has invested heavily in the Nook e-reader. It has faced tough competition from online retailers like Amazon.com and discount stores, with shoppers moving away from traditional books in favor of electronic books.

    2,750-year-old temple discovered in Israel

    An overhead view of the excavation site (Skyview/Israeli Antiquities Authority)Israeli archeologists have discovered the remains of an ancient temple that is nearly 3,000 years old and was once home to a ritual cult.

    "The ritual building at Tel Motza is an unusual and striking find, in light of the fact that there are hardly any remains of ritual buildings of the period in Judaea at the time of the First Temple," excavation directors Anna Eirikh, Hamoudi Khalaily and Shua Kisilevitz said in a statement released by the Israeli Ministry of Foreign Affairs.

    The temple remains were discovered at the Tel Motza site, located to the west of Jerusalem. The Israeli Antiquities Authority has been conducting excavation efforts at the site and says that along with the temple remains itself, the findings include a ?cache of sacred vessels? estimated to be 2,750 years old.

    "Among other finds, the site has yielded pottery figurines of men, one of them bearded, whose significance is still unknown," the statement from Khalaily and Kisilevitz reads.

    NBC?s Cosmic Log notes that the discovery was made during preparations for a new section of Israel?s Highway 1. Because of the number of historical sites and artifacts in and near Jerusalem, the Israeli government typically conducts similar archeological excavation efforts before beginning construction on major infrastructure projects.

    ?The walls of the structure are massive, and it includes a wide, east-facing entrance, conforming to the tradition of temple construction in the ancient Near East: The rays of the sun rising in the east would have illuminated the object placed inside the temple first, symbolizing the divine presence within. A square structure which was probably an altar was exposed in the temple courtyard, and the cache of sacred vessels was found near the structure.?

    The excavation directors said they will continue to examine the findings and conduct further digs while preparations for the highway construction continue.

    "The find of the sacred structure, together with the accompanying cache of sacred vessels, and especially the significant coastal influence evident in the anthropomorphic figurines, still require extensive research," they said.
